A 'Score Form'
is a method employed by the The Outcome Centre to determine your current status
of mobility, by filling in the forms on the hip score, ankle score, knee score,
and lysolm score pages, it will essentially determine a 'pain rating' for each
side of your body. These scores typically range from 1-100, with 1 being the
most painful, and 100 being the least painful.
Your scoreform is then added to a database and you are invited to return at
a later date to try your score again to see if your condition has improved.
If you are achieving especially low scores, then we would recommend you contact
your General Practitioner for advice.
